    • Contact Address with E-mail Address and Telephone Number(s)
    • Local Government of Origin
    • State of Origin
    • Nationality
    • Marital Status
    • Number and Ages of Children (if any)
    • Academic Qualifications Obtained with dates
    • Number of Publications (where applicable)
    • Years of Teaching and Professional Experience (where applicable)
    • Employment Records
    • Computer Training (Evidence enclosed)
    • Membership of a professional Bodies
    • Extra Curricula Activities
    • Names and addresses of three (3) Referees (Employer, Educational and Personal) who are to forward confidential reports directly to the Chairman, Gaya Polytechnic, Implementation Committee, with SUBJECT HEADING BEARING THE NAME OF THE APPLICANT, DEPARTMENT AND POST APPLIED FOR. Such referees should be able to attest to the Candidate's claim to high academic capabilities as well as to the Candidate's notable uprightness.
  • Valid Government approved ID (Driver's License, National ID, Biometric Page of International Passport) and Medical Certificate of fitness from Government hospital.
  • Two passport-sized photographs.

  Additional Information:

  • Candidates must have evidence of registration with any relevant professional bodies and be computer literate;
  • Applicants will be required to produce originals of their certificates and other relevant credentials if invited for interview, photocopies will not be accepted
  • Applicants who are applying to join the Public Service for the first time MUST be less than fifty (50) years of age at the time of resumption of duty
